We're running Matillion on an Azure VM and scaled the size to a D4s, but performing even basic functions in Matillion is so slow it isn't even usable. Are there any reasons why (even after scaling the VM) that this would perform so poorly?

Your best bet is to open a Matillion support case on this. It's going to take someone with intimate knowledge of how the VM was built and how the Matillion software was designed to function on the VM within Azure. There are a lot of variables that I don't think most of us could shed enough light on to understand what is going on.
